Abstract:Objective To measure left ventricular Tei index in second-third trimester fetuses with isolated pericardial effusion to investigate its relationship with gestational age,and to make a preliminary assessment of its clinical value.Methods Fifty-three second-third trimester fetuses without cardiac structural and rhythm abnormalities [28 male and 25 female;gestational age,18-23(20.3±0.4)week] were examined by fetal echocardiography because of isolated fetal pericardial effusion confirmed by color doppler ultrasound.Fetal echocardiography was performed and left ventricular Tei index was measured with tissue doppler imaging technique at less than <23,≥23-<27,≥27-<31,≥31-<35 and more than ≥35 weeks of gestation,respectively.The results were compared with normal controls.Results The sum of isovolumic contraction and relaxation time(ICT+IRT values) and Tei index in large pericardial effusion group were significantly greater than those in control group at less than <23 weeks of gestation(all P<0.05).The ICT+IRT values in middle pericardial effusion group were significantly less than those in large pericardial effusion group at less than ≥23-<31 and ≥31-≥35 weeks of gestation(all P<0.05).The ICT+IRT values in moderate and large pericardial effusion groups were significantly greater than those in control group and small pericardial effusion group at ≥23-<27,≥27-<31,≥31-<35 and more than ≥35 weeks of gestation(all P<0.05).The ICT+IRT values and Tei index in moderate pericardial effusion group were significantly less than those in large pericardial effusion group at ≥23-<27 and ≥27-<31 weeks of gestation(all P<0.05).Tei index in large pericardial effusion group was obviously greater than that in control group and small pericardial effusion group at ≥23-<27,≥27-<31,≥31-<35 and more than ≥35 weeks of gestation(all P<0.05).Tei index in moderate pericardial effusion group was obviously greater than that in control group and small pericardial effusion group at ≥27-<31 and more than ≥35 weeks of gestation(all P<0.05).Conclusion The left ventricular function is normal in second-third trimester fetuses with a small amount of isolated pericardial effusion.However,the left ventricular function decreased in fetuses with moderate or large pericardial effusion.Tei index measurement is a simple,fast,reliable and ideal indicators to evaluate fetal cardiac function and the prognosis of fetal pericardial effusion.
